This is my favourite procedures which has become very common in breast cancer surgery. It combines a wide local excision operation with a breast reductions operation. It is only suitable for patients with relatively large- sized breasts. Scenarios where this procedure is indicated:
Patients with symptomatic macromastia (excessively large breasts) causing distressing symptoms i.e. back and shoulder pain with small breast cancer
Patients with very large breast cancers ( > 5cm)
Patients with more than one (multicentric/multifocal) breast cancers

Patient with large ptotic (sagging) breasts with a large area of breast cancer.
