Perforator Artery Flaps – Case Study 1
Patient with two breast cancers in a moderately-sized breast.
  • 54-year-old lady who presented with a lump at the right lower outer quadrant (LOQ) of the right breast. Biopsy showed invasive ductal carcinoma, ER+ve, PR+ve, HER2-ve.
  • Mammogram, ultrasound and MRI showed another 10mm right upper outer quadrant breast cancer ( biopsy same as main tumour). The volume of breast tissue requiring excision was large and would usually require a mastectomy. However, the patient wanted to avoid a mastectomy and opted for a right localisation wide local excision + axillary clearance + LICAP perforator flap.
  • The lumpectomy operation excised 189g of breast tissue and the LICAP perforator flap was harvested from the skin and fatty tissue below the lower armpit and rotated in to fill the cavity left behind by the lumpectomy.
  • Final histology showed tumour 1 (right upper outer quadrant) 9mm G1 IDC with intermediate grade DCIS (total tumour size 9mm). Tumour 2 right lower outer quadrant – 60mm G1 IDC with intermediate grade DCIS (total area 21mm). All margins were clear by more than 1 cm.
  • Oncotype Dx test indicated that patient did not require chemotherapy. She received right breast radiotherapy and then started on Letrazole.
